About this good practice
The Ability research project, funded in Italy within the Smart Cities and Smart Communities funding program, aims at developing and testing the efficacy and the impact of a Personal Smart Health Community able to provide innovative trajectories for people with cognitive impairment, putting them at the core of a continuous and intertwining treatment and support from both formal (e.g. physicians) and informal (e.g. near relatives) caregivers, with special focus on home-based care. The programme is focused on elderly people, who may suffer from a long-term residence in hospitals.
Within this framework of the Ability project the investigators test the efficacy of the home-based motor-cognitive rehabilitation program delivered with two different approaches: the Ability platform versus the usual care program.
In line with the TITTAN third thematic area, the programme aims at improving patients ‘quality of life through the use of new technologies, since its participant-tailored and technology-enhanced platform delivers a tablet-based intensive motor and cognitive activities and it allows patients to have their parameters monitored remotely.

Evidence of success
The success of this research project is guaranteed by the multidisciplinary approach given by a consortium of public and private entities involved. Fondazione Don Gnocchi is in fact collaborating for its implementation with different partners, including academic partners (University of Milan Bicocca, Politecnico of Milan) and private partners (Telbios Spa, Imaginary S.r.l. )
Potential for learning or transfer
The main aim of the project has been the elaboration and implementation of a “Smart care Community”, which means a comprehensive system that encompasses health and wellbeing for elderly people at risk of dementia. It provides care, remote-monitoring, engagement.
Public administration will eventually benefit from the reduction of hospitalisation , as well as from a better rehabilitation of patients
