CHANGES TO THE HOME CARE SERVICES AND INTEGRATION OF INNOVATIVE SERVICES

About this good practice
Corresponds to a set of free initiatives, related to the promotion of physical activity and cognitive stimulation for a specific population receiving the Domiciliary Assistance Service - elderly people at risk of physical frailty *, who are at home, regardless of age and sex, have in this project a fundamental instrument to maintain or improve their functional physical fitness, and consequently the quality of life.
* Fragile Elderly - Is the individual with physical and functional limitations, chronic diseases, or who are very debilitated, and who is more susceptible to accidents / illnesses. According to some researchers the fragility has two aspects: a) there is a decrease in physiological reserves in multiple areas, such as endurance, flexibility, muscle strength, balance, reaction time, coordination, muscle functioning, hearing, vision, nutrition, and cognitive functioning; B) and a predisposition to suffer adverse effects in situations of risk.
Elaborate an exercise plan (exercises for the development of functional physical fitness and exercises of cognitive stimulation), called "Book of Prevention of Physical Fragility" - this document, created by the Psychomotrician, will allow the person himself to have the plan of Following exercises, while at the same time recording their physical and functional evaluation and evaluating their progress. The Occupational Helper will have to supervise, assist and advise the elderly how to carry out the exercise plan.

Resources needed
A multidisciplinary team made up of a Psychomotrician, Social Worker and Occupation Helper .Printing of the leaflets for the dissemination of the project (during the project), printing of the "Carnivals of the Prevention of Physical Fragility", acquisition/preparation of intervention material kits
Evidence of success
On this date, the project is in the 4th phase of implementation - Implementation of the Cognitive Stimulation and Functional Development Program, o fare all of participants are 100% very satisfied.
Potential for learning or transfer
Working methodologies are fully transferable based on the replication of the network of partners in the social intervention sectors, with the appropriate adaptations to the different local realities.
Allows to strengthen and diversify the home support service with the extension of the provision of care and services, in particular introducing of animation and socialization activities at home.
It allows working with patients in their homes, not having to travel to public institutions, reducing costs.
