Discover the policy instruments that the partners of this project are tackling.
A means for public intervention. It refers to any policy, strategy, or law developed by public authorities and applied on the ground to improve a specific territorial situation. In most cases, financial resources are associated with a policy instrument. However, an instrument can also sometimes refer to a legislative framework with no specific funding. In the context of Interreg Europe, operational programmes for Investment for Growth and Jobs as well as Cooperation Programmes from European Territorial Cooperation are policy instruments. Beyond EU cohesion policy, local, regional, or national public authorities also develop their own policy instruments.
The Zemgale Planning Region Development Programme for 2021-2027 is a strategic document guiding the development of the Zemgale region. Among its priorities, P2 - Social Inclusion and Promotion of Health is particularly relevant to the FABCOM project and aims to addres to following topics:
-Sustainable Development of Social Services: Improve accessibility, diversity, quality, and relevance of social services to meet the needs of the region's population.
-Health Participation: Enhance the region's population's participation in maintaining their health by promoting access to health services and implementing health promotion measures.
-Social Inclusion: Increase opportunities for people at risk of social exclusion to participate in the economy, education, culture, and social activities. This is achieved by improving environmental accessibility, reducing stereotypes, and promoting social inclusion.
Other relevant priorities include:
-P6 - Modern and Accessible Service System, which emphasizes developing a modern service system that is easily accessible to all, and
-P8 - Civil Society and Active Communities, which promotes active community engagement and civil society. Horizontal priorities such as capacity building, digitization, compliance with green principles, and cooperation support these efforts.

The political strategy of the Diputación Provincial de Jaén is anchored in the II Strategic Plan of the Province of Jaén, which outlines the key action lines for decision-making. Specifically, Chapter 8, titled "Facts and Challenges for Jaén: A Province for Coexistence and Social Well-being," addresses the main objectives relevant to the FABCOM project.
The Diputación Provincial de Jaén focuses on enhancing its policy instrument through improved coordination and management of community social interventions. This improvement aims to achieve effective integration with information systems at regional (Andalucía) and European levels. By ensuring that data and evaluations are exportable and comparable with other national and European regions, Jaén can align its social well-being initiatives with broader strategic goals.
This strategic approach supports FABCOM's objectives by promoting social inclusion, enhancing the quality of social services, and fostering data-driven decision-making for community-based care models. The integration with regional and European information systems facilitates benchmarking and sharing best practices, contributing to the overall effectiveness and sustainability of social interventions in Jaén.

The Silver Małopolska 2030 Strategic Programme is one of the instruments of the Małopolska Region’s ageing policy aimed at creating conditions to extend professional and social activity of senior citizens as well as to enable independent, healthy, and safe ageing of the inhabitants. The Programme’s structure based on four priorities reflects the wide range of needs and challenges as well as respective recommendations and actions. The priorities are the following: 1. Health and care, 2. Social activation, 3. Vocational activation, 4. Space and housing. The project goal of improving regional community-based and family-based care is in line with the first Programme priority: “Health and care” and its second intervention direction: “Improving access to high-quality health and social services for independent living”. The “Health and care” priority responds to the problems of increasing demand for senior-oriented services, significant shortage of medical staff and carers as well as assistive infrastructure to support active and healthy ageing. Within the second intervention direction, the following task is applicable to the project goal: Task. 1. Accessible care – development of community care services, day care and therapy for senior citizens. The above policy line corresponds to the provisions of the “Małopolska 2030” regional development strategy related to improving life quality of the Małopolska inhabitants, in particular families and senior citizens.

The main objective of this policy instrument is to implement a provincial policy to increase the health resilience of rural areas. In other words, to make local public authorities more resilient by visualising, discussing, naming, making decisions and, where possible, tackling joint challenges for the village. Part of this resilience is committing to a caring neighbourhood, where the conditions are met that allow people to (continue to) live comfortably in their home or familiar neighbourhood. It is a neighbourhood where young and old know and help each other. Quality of life is central; facilities and services are accessible to everyone. Everyone feels good and is helped, regardless of their support needs. Our current way of working in our “Caring Villages” in the Province of Antwerp is to strengthen the resilience of villages and neighborhoods regarding to the social fabric and the offer of care by:
- Social cohesion;
- Strong local and supra-local partnerships;
- Better detection and guidance to the wright health care providers;
- To strengthen the health care providers and primary care.

The Specific Objective (SO) of ROP Sterea Ellada 21-27 is “RSO4.5. “Ensuring equitable access to healthcare and strengthening the resilience of health systems, including primary care, and promoting the transition from institutional care to family and community-based care”. RSO4.5. aims to strengthen and modernize medical technology equipment, remote care, and support health structures with an emphasis on strengthening Primary Health Care and upgrading long-term care services in order to reduce the burden on secondary care, especially in areas with high concentrations of vulnerable populations. n addition, the creation of Supported Living Homes will strengthen the national deinstitutionalization policy as mentioned in the National Strategic Reference Framework 2021-2027 in the Policy Priority "Promoting deinstitutionalization and rehabilitation". The interventions are in line with the directions of the European Pillar of Social Rights action plan for the promotion of health, the provision of care and social protection. The actions will follow the National Action Plan for primary care.
The interventions of Specific Objective 4.v (ERDF) are part of the strategic Priority 4.1 and the categories of actions envisaged are the following:
• Development of health infrastructure
• Upgrading and expansion of healthcare equipment
• Development of an intelligent epidemiological surveillance and environmental health monitoring system based on wastewater epidemiology

Decentralized Target Planning (DTP) is a regional policy instrument developed by Pro Arbeit guiding labor market integration and workforce development in Kreis Offenbach. It was created in collaboration with the Hessian Ministry for Labor, Integration, Youth and Social Affairs. It establishes employment priorities, funding strategies, and targeted support measures to address local labor market challenges.
A core focus is to improve labor market access for disadvantaged groups (long-term unemployed, women, migrants). The instrument emphasizes tailored employment programs, vocational training, and support services to enhance job placement opportunities.
It also addresses sectoral workforce shortages, particularly in health and social care, by promoting upskilling and reskilling initiatives. This helps to ensure that the local labor force meets the growing demand for skilled professionals.
Recognizing the importance of digital transformation, the instrument integrates strategies for digital skill development to equip jobseekers with competencies relevant to modern workplaces.
DTP is updated annually to reflect changing labor market conditions and aligns with Hessian and national employment strategies. It also supports the leveraging of external funding, including ESF+ and other EU funding, to finance employment.
By providing a structured and adaptable framework, this instrument plays a crucial role in shaping sustainable employment pathways and fostering social inclusion.

The Guide and Protocol for Providing Curative Services for Persons with Disabilities and Young People with Severe Non-Communicable Diseases is a policy instrument aimed at strengthening regional capacities for personalized, community-based care. It supports the EU's transition from institutional to community-based services, focusing on accessibility and quality, particularly in rural areas.
Key features include:
Standardization of healthcare services – Defines protocols for professionals, ensuring quality and consistency.
Improved accessibility – Mobile healthcare teams and home care enhance support for vulnerable groups.
Transition to community-based care – Facilitates home-based care, reducing institutional reliance.
Training of healthcare professionals – Ensures a multidisciplinary approach through specialized education.
This instrument aligns with national strategies, notably the Strategy for the Protection of Persons with Disabilities from Discrimination and Promotion of Equality (2022–2027), reinforcing its role in Montenegro’s healthcare policy. Its priority is improving patient outcomes while reducing the healthcare system's financial burden through sustainable services.
