Hybrid Care for Older Social Inclusion

HYCARES (HYbrid CARE for older Social inclusion) is about building an institutional and organisational framework among Local Authorities, public and private care providers and citizens that will support the transition from a basic home care for older people towards a community nursing intervention that will embrace the inner circle of frail older, living alone ato home, making use of ICT solutions to adopt a hybrid care model (face to face and remote support).
The project will make use of AI to build up a digital twinning health care modelling that will allow social and health care services using a forecast model that will reduce the risk of cognitive and functional decline of the elderly.
